How to use the report and finding register

Understand the report prompts, finding fields, source references and change history.

The Word-compatible report carries the narrative. The spreadsheet/CSV register makes every finding traceable. Neither is a legal opinion or an approval system. The worked report is the readable HTML/Markdown alternative to the completed Word file.

The finding fields

Finding register fields and their purpose
Field What belongs here
item_id A stable ID such as F01; keep it when the wording changes. Q01 identifies an inquiry, not a low-risk finding.
item_type finding or inquiry; missing information can remain an inquiry.
priority A proposed label under the client’s stated criteria, not a universal risk score.
priority_reason Why this issue matters to this decision; identify the instruction or dependency.
source_ids IDs separated by |; every ID must appear in the source index.
locators Exact bracketed paragraph labels, separated by |, with at least one for each source.
finding What the supplied evidence supports, attributed where necessary.
significance A possible transaction consequence, separate from the fact.
uncertainty Missing, contradictory or unverified material and limits on the conclusion.
next_action A specific request or proposed response, not a claim that it is authorised.
owner The responsible role; fictional roles do not appoint real people.
status Whether the request/decision is open; receipt of a file is not approval.

Files and safe editing

The blank workbook contains no example finding rows or sources. It includes Read me, Findings, Sources and Change log sheets, with headers and blank working rows. The completed workbook contains the four baseline entries. Cells are text; there are no formulas, macros or external workbook connections.

CSV files do not preserve formatting, text types or access controls. Import all columns as text, keep UTF-8 encoding and verify the | separators. Do not paste formulas or confidential matter data into a test copy. Sort the entire table, including IDs and source fields, not an isolated column. Reopen an exported copy and trace F02 to its sources.

In the report, replace every prompt and reconcile the executive summary after editing a finding. Do not keep template examples as if they were your matter’s facts. An Open cell is a reminder, not a technical prohibition on sending the file.

Version and change history

Keep the article, source pack, report, register and checklist at v0.1.0 for this exercise. Snapshot the baseline before attempting the update. The blank change log records item, field, old value, new value, source locator, reason and human approval status. Log every changed field, including sources and priority reasons. An unchanged row needs no artificial edit.

These files have no automatic link to a document room or live approval service. Only tested application environments listed in the package QA record are claimed; human usability and qualified review remain pending.
